E420: 50% premium within 24 hr when ISS >15. Applies to basic/time units and consult/assessment fees. Any single score of 4 qualifies.
Head / Neck
4 Brain stem herniation, DAI, contusion/epidural/subdural >30 cc; unconscious >6 hrs; pharyngeal perforation; complete/incomplete cord injury.
3 Unconscious >1 hr or GCS <9; carotid injury; laryngeal perforation; pharyngeal tear; cervical fracture.
2 Unconscious <1 hr or GCS 9-14.
Face
3 LeFort 3.
2 LeFort 1 or 2; zygoma/orbit fracture.
Chest
4 Open chest wound; aortic tear; bilateral pulmonary contusion; tension pneumothorax; flail chest.
3 Hemo/pneumothorax; ruptured diaphragm; cardiac/pulmonary contusion; tamponade; >3 unilateral rib fractures; T-spine fracture.
Abdomen
4 Aortic tear; bladder rupture; renal hilum; liver/spleen burst or disruption.
3 Bowel perforation; kidney laceration; liver/spleen laceration >3 cm; L-spine fracture.
Extremity / Pelvis
4 Major pelvis with retroperitoneal hematoma.
3 Closed femur; open longbone; SI or symphysis separation; tibial plateau.
2 Any longbone fracture.
External
4 Burn >30%; >20% on face/hand/genitalia; COHb >20%; rectal temp <30.
3 Burn >20%; >10% on face/hand/genitalia; COHb <20%; rectal temp <32.
